 You may get your Registration Form on our website at , renap fb or at the RENAP main office at 4A 4th Flr. Hospital Bldg, Philippine Heart Center, East Ave, Quezon City and pay the necessary amount . In order to avoid delays in processing your registration, please write or type clearly the information required including payment information that have been made. Use a separate form for each delegate. Ask for your receipts and ask for other information needed during the Convention.

 IMPORTANT REMINDER: DO NOT FORGET TO BRING your receipt on the day of the Convention.
 FOR COMPANY CHEQUE: RENAP is a non-profit organization and does not accept payment with tax deductions.

 Registration for the 2017 RENAP Annual Convention is a first come first serve basis. When the quota is reached to 700 participants, NO ONSITE / ON THE DAY Registration will be entertained.
 Please check our website and facebook account for the census update starting October . If 700 participants were already registered, we will be announcing the closing of registration.

 To avail of the MEMBERSHIP RATE, the individual shall provide a proof that he/she has been an active member of RENAP.
 No instant membership shall be allowed just to avail of the membership registration rate.
 Registration fee is inclusive of kits, Congress ID’s, meal stubs; one certificate of attendance accredited by PRC – Board of Nursing for credit units.
 One receipt shall be issued under a principal name (person / company or hospital)
 A Registration Form duly accomplished by each of the group shall accompany the payment.
 For INSTITUTIONS who cannot meet the prescribe date of registration payment, you may submit a request letter duly signed by the Director of Nursing, to avail the participant’s slot. Please indicate the name and details of the participants. The letter should be received not later than OCTOBER 31, 2017, otherwise there will be no guarantee of the request’s approval.
 Registration Fee includes of Convention kits, Convention ID’s, meal stubs, individual certificates of attendance accredited by PRC – Board of Nursing for credit units.
In the event that the Registered Participant is unable to attend the conference, your registration is transferable at no additional charge. Contact the Secretariat if you wish to transfer your registration to another person. Please accomplish a letter of transfer request and fill up another registration form for the person you wish to transfer your registration and attach your registration form and payment receipt.
Transfer request should be made at least FOUR WEEKS prior to the convention.
*STRICTLY NO TRANSFER OF REGISTRATION will be accepted on the day of the Convention
